ความสามารถของเภสัชกรในงานดูแลผู้ป่วยแบบประคับประคองตามความคาดหวัง ของทีมสหสาขาวิชาชีพ

Abstract

การศึกษาความสามารถของเภสัชกรในงานดูแลผู้ป่วยแบบประคับประคองตามความคาดหวังของทีมสหสาขาวิชาชีพครั้งนี้ เป็นการศึกษาแบบพรรณนาในรูปแบบเชิงส่ารวจ แบ่งการศึกษาออกเป็น 2 ระยะ โดยระยะที่ 1 ศึกษาความคาดหวังของทีมต่อบทบาทและคุณลักษณะของเภสัชกรในการท่างาน palliative care ซึ่งเก็บข้อมูลด้วยการสัมภาษณ์สมาชิกในทีม palliative care ที่คัดเลือกแบบเจาะจงจ่านวน 2 ทีม ใช้การวิเคราะห์เชิงเนื้อหาให้ได้มาซึ่งบทบาทและคุณลักษณะของเภสัชกรในงาน palliative care ส่วนระยะที่ 2 ศึกษาระดับความสามารถของเภสัชกรที่ปฏิบัติงานในโรงพยาบาลระดับตติยภูมิในการท่างาน palliative care ตามบทบาทจากความคาดหวังของทีมที่ได้จากการศึกษาระยะที่ 1 ด้วยการส่งแบบสอบถามไปยังเภสัชกรที่ปฏิบัติงานในโรงพยาบาลระดับตติยภูมิจ่านวน 858 คน ใช้วิธีการสุ่มแบบแบ่งชั้นโดยใช้สัดส่วน และวิเคราะห์ข้อมูลด้วยโปรแกรมส่าเร็จรูปโดยใช้สถิติเชิงพรรณนา และเชิงอนุมานโดยใช้ T-Test กับ One Way Anova ข้อมูลจากการสัมภาษณ์พบว่าสมาชิกในทีมและเภสัชกรเห็นตรงกันว่าเภสัชกรเป็นวิชาชีพที่จ่าเป็นต่อทีม palliative care โดยเห็นว่าด้านการดูแลที่เภสัชกรจะเข้ามาช่วยเหลือทีมได้ดีที่สุดคือ การดูแลด้านร่างกาย ซึ่งระบุบทบาทหลักในงานบริบาลทางเภสัชกรรมทั่วไป เช่น การแนะน่าขนาดยา ผลข้างเคียงจากยา อีกบทบาทหลักคือเรื่องการจัดการอาการปวด โดยเน้นเรื่องการสร้างความเข้าใจในการใช้ยาแก้ปวดกลุ่ม Opioids และคุณลักษณะที่ส่าคัญที่ทีมต้องการจากเภสัชกรคือ ทัศนคติที่ดีต่องาน รองลงมาคือการท่างานเป็นทีม ส่วนข้อมูลจากแบบสอบถามพบว่าความสามารถของเภสัชกรในการท่างาน palliative care ตามบทบาทที่ทีมคาดหวังจากคะแนนระดับความสามารถเต็ม 10 นั้น เภสัชกรต่างเห็นว่าตนเองมีความสามารถในการท่างานโดยรวมอยู่ในระดับมาก ( X = 6.50, SD = 1.73) เมื่อพิจารณารายด้านก็พบว่าด้านการดูแลที่เภสัชกรเห็นว่าตนเองมีระดับความสามารถในการดูแลอยู่ในระดับมากที่สุดคือ ด้านร่างกาย ( X = 7.22, SD = 1.49) ส่วนด้านการดูแลที่เภสัชกรเห็นว่าตนเองมีระดับความสามารถอยู่ในระดับมากคือ ด้านสังคม ( X = 6.44, SD = 2.00) และด้านการดูแลที่เภสัชกรเห็นว่าตนเองจะมีระดับความสามารถอยู่ในระดับปานกลางคือ ด้านจิตใจ ( X = 6.29, SD = 2.00) และด้านจิตวิญญาณ ( X = 6.05, SD = 2.14) โดยกลุ่มบทบาทงานที่เภสัชกรเห็นว่าตนเองจะมีระดับความสามารถในการดูแลมากที่สุดส่วนใหญ่จะเป็นงานที่เกี่ยวกับงานบริบาลทางเภสัชกรรม เช่น การแนะน่าขนาดยา ปฏิกิริยาระหว่างยา ผลข้างเคียงจากยา รองลงมาคือกลุ่มบทบาทงานเรื่องยาที่เฉพาะเจาะจงกับการดูแลแบบ palliative care เช่น การประเมินทัศนคติต่อการใช้ยาแก้ปวดกลุ่ม Opioids ของทั้งผู้ป่วยและญาติ การแนะน่าขนาดยา Naloxone และบทบาทงานที่เภสัชกรเห็นว่าตนเองจะมีระดับความสามารถในการดูแลน้อยที่สุดก็คือ งานที่เกี่ยวกับการดูแลผู้ป่วยในด้านจิตใจและจิตวิญญาณ เช่น การช่วยค้นหาปัญหาที่ยังค้างคาใจของผู้ป่วย ส่วนคุณลักษณะที่ดีของเภสัชกรในการท่างาน palliative care ในมุมมองของเภสัชกรเองคือ เรื่องขององค์ความรู้และรองลงมาคือความสามารถในการท่างานเป็นทีม1 This ability in pharmacist’s palliative care according to expectations of multidisciplinary team study case was studied by descriptive survey form, divided in two periods. The first period, it was studied about team’s expectation in pharmacists’ role and characteristic during palliative care, selected two specifically team by using content analysis to obtain. The second period, it was studied about ability of pharmacist who worked for palliative care in the tertiary hospital, accorded to the role of team from the first period by handing out a questionnaire to 858 pharmacists, then stratified random sampling was performed and the data was analyzed with software by using T-test and One way ANOVA from descriptive and inferential statistic. From the data by interviewing, it was found out that the team’s member and pharmacist agreed that pharmacist is the necessary occupation of the palliative care team and also it was found that the best support from pharmacist to the team was physical care, assigned in the main role of general pharmaceutical care; such as recommended the dosage, side effect from medicines. Another main role was to manage the pain by focusing on the understanding of how to use Opioids painkiller, and the important characteristics the team needed from the pharmacist were good attitude in working and team work. The data from questionnaires showed that the ability of pharmacist in palliative care, rolled in team’s expectation by themselves, were in high level ( X = 6.50, SD = 1.73), when considering for individual care filed, the pharmacists thought they had very high level caring ability in physical ( X = 7.22, SD = 1.49), and in the field they thought that they had in high level was social care ( X = 6.44, SD = 2.00), in the mental field they thought they had in normal level ( X = 6.29, SD = 2.00) and in the spiritual field ( X = 6.05, SD = 2.14). By the role in team, the pharmacists thought that they had care ability in the highest level was pharmaceutical works; such as recommending the dosage, the reaction between medicines and side effect from the medicines, in the next lower levels were the using of specific medicine and palliative care; such as evaluating the patient’s attitude in using Opioids painkiller, recommendation on Naloxone dosage, and the lowest level that they thought they had was the mental and spiritual cares; such as finding the disturb problem of the patients. The good characteristic of the pharmacist view in palliative care was the knowledge and team works.
