COMPLEX ORDINARINESS: CONSTRUCTION OF MEANINGS AND RESIDENTS' EXPERIENCES OF PARACETAMOL USAGE: A CASE STUDY OF A PROVINCE IN THE CENTRAL REGION

Abstract

This qualitative research aims to study a set of experiences and understanding of paracetamol uses and meanings among people living in the central region of Thailand. The research is done on 43 informants with various methodologies, which are: 1) Observations on group discussions of the people who experienced using paracetamol., 2) Insightful interviews with key informants - chosen by the method called “snowball sampling”, who relied heavily on paracetamol in one or several aspects of their life. In case a key informant gives inconsistent information - either inconsistent with their previous interviews or with observed facts, I will extend the observation to the surrounding people such as relatives, related shop owners, and medical staff. I apply the concept of “social construction of reality” to organize the information. With content analysis and thematic analysis, I can see that people tend to rely on their direct experiences with paracetamol to designate the approach and amount they should take. This results in a diversity of paracetamol use and comprehension on it: 1) Paracetamol as a tool to manage life., 2) Paracetamol as a tool to prevent problems in life., 3) Expansion of practice and use of paracetamol., and 4) Dose adjustment depending on the context of life. It appears that people also define new meanings for paracetamol, which diverge from the typical meaning known among medical professions, based on 1) Their personal introduction to the medicine, 2) Factors causing them to use the medicine, 3) Presumption of effectiveness, and 4) Contemplation between personal and public facts. This study shows that paracetamol, once put in a hand of people, transforms from a medical substance invented to ease and cure illness to a cultural object that helps deal with problems and difficulties in life, based on each person’s perception and experiences. Paracetamol, therefore, is a typical medicine with complex background.

การวิจัยเชิงคุณภาพชิ้นนี้มีวัตถุประสงค์เพื่อศึกษาชุดประสบการณ์ และการรับรู้ความหมายของการใช้ยาพาราเซตามอลในมุมมองของประชาชน ในพื้นที่จังหวัดแห่งหนึ่งในภาคกลาง โดยมีผู้เข้าร่วมการศึกษาทั้งหมด 43 คน และใช้การเก็บรวบรวมข้อมูลหลายวิธี ได้แก่ 1) การสนทนากลุ่มพร้อมการสังเกต ผู้ที่มีประสบการณ์ใช้ยาพาราเซตามอล 2) การสัมภาษณ์เชิงลึกพร้อมการสังเกตผู้ให้ข้อมูลหลักที่มี ประสบการณ์ในการใช้ยาพาราเซตามอลเพื่อรักษาตนเองอย่างเข้มข้นในแง่ใดแง่หนึ่ง หรือหลายแง่ ด้วยวิธีการคัดเลือกแบบลูกโซ่ (snowball sampling) กรณีที่ข้อมูลที่ได้จากผู้ให้ข้อมูลหลักไม่สอดคล้องกับข้อมูลที่ได้จากการสังเกต หรือการให้ข้อมูลแต่ละครั้งของผู้ให้ข้อมูลหลักไม่ตรงกัน ผู้ศึกษาได้ดำเนินการ เก็บข้อมูลกับผู้ให้ข้อมูลสนับสนุนที่แวดล้อมผู้ให้ข้อมูลหลักเพิ่มเติม ซึ่งผู้ให้ข้อมูลสนับสนุน ประกอบด้วย ญาติ เจ้าของร้านชำ บุคลากรทางการแพทย์ที่อยู่ในโรงพยาบาลหรือร้านขายยา สำหรับการวิเคราะห์ข้อมูลจะใช้แนวคิดเรื่องการประกอบสร้างความจริงทางสังคมเป็นเครื่องมือในการวิเคราะห์ โดยข้อมูลที่ได้ จะถูกนำมาจัดกลุ่มแบ่งประเภท วิเคราะห์เชิงเนื้อหา (content analysis) และสร้างข้อสรุปการวิจัย ด้วยการวิเคราะห์ธีม (thematic analysis) ผลการศึกษาพบว่า ผู้คนใช้ชุดประสบการณ์ของตนเองกำหนดวิธีการใช้ยาพาราเซตามอล ส่งผลให้มีรูปแบบการใช้ที่แตกต่างหลากหลาย ได้แก่ 1) การใช้ยาพาราเซตามอล ในฐานะเครื่องมือจัดการชีวิต 2) การใช้ยาพาราเซตามอลในฐานะช่วยป้องกันปัญหาในชีวิต 3) การขยาย ข้อบ่งใช้ของยาพาราเซตามอลผ่านประสบการณ์ของผู้ใช้ และ 4) การปรับขนาดการใช้ยาพาราเซตามอล ตามบริบทชีวิต นอกจากนี้ยังมีการให้ความหมายของยาพาราเซตามอลที่แตกต่างไปจากความหมาย ในมุมมองทางการแพทย์ ประกอบด้วย 1) จุดเริ่มต้นที่ผู้คนใช้ยาพาราเซตามอลในการรักษาความเจ็บป่วย 2) แรงผลักดันให้ผู้คนใช้ยาพาราเซตามอล 3) การผูกโยงความหมายเรื่องความแรง และประสิทธิภาพของ ผู้คน และ 4) การที่ผู้คนมีการต่อรองระหว่างชุดความจริงของเขากับชุดความจริงอื่น โดยการศึกษานี้ แสดงให้เห็นว่า เมื่อยาพาราเซตามอลได้เข้ามาอยู่ในมือของผู้คน ยาพาราเซตามอลจะมีการเปลี่ยนผ่านจาก วัตถุที่ใช้บำบัด บรรเทา หรือรักษาความเจ็บป่วย กลายมาเป็นวัตถุทางวัฒนธรรม และถูกนำมาใช้จัดการชีวิต ตามบริบทและประสบการณ์ของแต่ละบุคคล ยาพาราเซตามอลจึงเป็นยาธรรมดาที่มีความซับซ้อน อยู่เบื้องหลัง
